When you submit information through our website inquiry forms, you are not yet a patient of Epic Revive and a formal patient-provider relationship has not been established. Information submitted through our website forms is not protected health information (PHI) under HIPAA at the time of submission.

Once you become a patient of Epic Revive, your health information will be protected under a separate Notice of Privacy Practices, which will be provided to you at the time of your first appointment, as required by HIPAA.
